REGULATIONS AND CONDITIONS FOR THE USE OF ST.JOHN AMBULANCE HALL AND PREMISES
APPLICATIONS
Applications for the use of the St. John Ambulance hall or premises must be made in the prescribed form obtainable from the office.

DEPOSIT WILL BE FORFEITED IF THE KOLAM, BANANA TRUNK,BALANCE FOOD ,PAPER CUPS & PLATES NOT CLEARED OR THROWN INTO THE DRAINS ALSO IF THERE ARE ANY DAMAGES
TERMS OF PAYMENT
The contribution as set out above for each proposed period of use must be forwarded with the application form together with full payment of RM1400.00 and RM 100.00 deposit. The deposit of RM 100.00 will be forfeited in full if there are breakages, damages to the building fittings / equipments, kolam not cleared, banana trunk not thrown and balance food (leftovers) not cleared or thrown into drains. Otherwise will be refunded one week after the expiry period of use. The full deposit will be forfeited if it is not claimed within 90 days from expiry of the period of use.
PENALTY OF CANCELLATION
In the event of a booking being cancelled,
- Only Air-Con & Canopy charges will be refundable.
CHANGE OF DATE
In the event of change of booking date
- Change of Date is allowed only once, upon availability of the hall.
HALL AND STAGE LIGHTING
Only the SJAM caretaker can operate the stage lighting and other hall lighting.
EXPIRY OF THE PERIOD OF USE
The applicant shall ensure that the hall is cleared of all persons, equipments, belongings etc NOT later than 1 hour after the expiry of the used period and in default the deposit will be forfeited.
CHARITY FUNCTIONS
Performances in aid of Charity may be given discount on the contribution, as the committee deems fit. All applications should be made in writing at least two months before the function to the Hon Secretary, St.John Ambulance State of Perak.
RIGHT OF ENTRY
The Management Committee and the staff of the SJAM Perak on duty shall be allowed to access at all times to inspect the hall or carry out other duties.
ELECTRICITY & WATER SUPPLY
The management Committee will not be held responsible or liable for any failure of electricity power or water supply.
ADVERTISEMENT
Applicants are not allowed to display any photographs, banners other advertisements in or around the SJAM Perak hall or premises without the approval of SJAM Perak Management and /or Dewan Bandaraya Ipoh.
SEATING CAPACITY
The seating capacity is approximately
- 800 persons theatre style.
- 8 persons each table.
LOSS DAN DAMAGES
The applicants shall be held liable for any loss or damages to any article, equipment or fixtures of SJAM Perak Building during the period of use. The Management Committee of SJAM Perak shall not be responsible for any loss or damages to any property belonging to whosoever using the hall.
POLITICAL ACTIVITIES
The applicants are not allowed to use the premises for POLITICAL ACTIVITIES.
PUBLIC SAFETY OR ACCIDENT
The applicant shall be held responsible and liable for the safety and the convenience of the public and shall take out any insurance cover as may be deemed necessary to meet such contingencies. The Management or the Society shall not be responsible nor liable in any way whatsoever for any accidents resulting in deaths or injuries of any nature to members of the public arising from their attendance (whether as paying guest or otherwise) at Functions / Promotions / Pesta / Expo / Meeting / Entertainments / Seminars / Conference / Luncheons / Dinners /Dancing / Exhibitions etc held in St. John Ambulance Malaysia Negeri Perak Hall due to negligence on the part of the Applicants.
LICENCE
The applicant shall be responsible for obtaining all necessary licences from relevant authorities to carry out his / her function / activity and in the maintenance of peace and order. (Photocopy of the Licence shall be submitted to the Management Board)
CANOPY
All canopy should be placed at the back portion of the hall. NO CANOPY ARE ALLOWED TO BE PLACED AT THE FRONT PORTION OF THE HALL.
FOOD CATERERS
The applicant has to take all responsibilities to ensure that his/her Food Caterers take away all waste / rubbish / leftovers immediately after the function, failing which the Deposit of RM 100.00 will be forfeited.
GENERAL
The Management reserves the RIGHT :-
- To refuse any application without giving any reasons
- To limit period of use as may be considered necessary
- To terminate with immediate notice where a serious breach of the rules has be committed.
- To cancel an approved application without having to give prior notice under extraordinary circumstances and shall not compensate the applicant for any loss or damage as a result of the cancellation. However, the Management shall refund only the rental and deposit of the hall paid by the applicant..
